Your pet is very important to us and every effort will be made to make your animal’s grooming experience as safe and pleasant as possible. Please read and acknowledge acceptance of each section by checking every box. * Prices Vary: I acknowledge that all prices listed or quoted are only estimates and the final price will depend on the condition of the coat and/or extra time needed for extreme behaviors or health problems. Any discovery of parasites, ear mites, or matting shall be treated as an add-on service and owner agrees to the extra charge. I understand and agree to pay in full for any missed grooming if not cancelled within 24 hours of the appointment. Owner Responsibility for Pet Medical Care: Safety comes first and although we are cautious, problems may occur because of age, health, or behavior. If any serious injuries or serious health problems occur, Creekside will notify owner for instructions and if unavailable, Creekside will seek immediate veterinary care for your pet at owner’s expense. Hold Harmless: Grooming can expose a hidden medical problem or aggravate a current one. Owner agrees not to hold Creekside Pet Boarding responsible for any injuries or irritations which might result from the grooming process. Aggressive or Overly Reactive Dogs: Creekside reserves the right to refuse service or stop a groom in progress if your dog may pose a threat to themselves, other pets, or staff, whether it be an aggression or health problem. Extra charges may apply if two groomers are necessary. Remember, regular monthly grooming helps ease the fear or aggression issues. Owner Responsibility: Owner agrees to be solely responsible and liable for all acts of their pet(s). This may include but is not limited to injury or death to their dog, another dog, staff members, or other clients. Owner responsibility extends to damaged property. Hold Harmless; Aged or Special Needs dogs: Owner is aware that the process of grooming for older or special needs dogs may become mentally and physically stressful. This may cause latent, unknown, active or inactive conditions such as arthritis, bone, joint or surgical sites to become active or inflamed, and unknown active or inactive heart, kidney, or liver disorders to become active, and can result in illness, seizures, or the death of the dog. Although Creekside Pet Boarding will take special care in the grooming of the dog, owner acknowledges that the stress of grooming may initiate immediate or latent medical problems. Owner agrees not to hold Creekside Pet Boarding responsible for reactions to grooming Dogs should be dropped off by 10 am and picked-up no later than 3 pm. Special requests to be bathed earlier or ready at a specified, earlier time will incur a “priority client” fee of $20 extra. If you are not satisfied with your pet’s grooming, we will do all we can to rectify the problem. The options are re-bathing, re-brushing, or a limited credit toward your next appointment.
